Field Trip Checklist

Note: All forms must be completed and submitted to the Architecture Department office by the required deadlines, as stated on our Field Trip Guidelines web page. Please consult the website for more information on faculty responsibilities in leading an off-campus field trip.

For Local 1-2 Day Trips

  1. Faculty must complete the following:
    • Travel Request Form (1A)
    • Field Trip Itinerary
  2. Students must complete the following:
    • University Field Activities Notification
    • University Activity Release Agreement (only necessary if field trip involves unusual risks)

For Extended (2+ days) / International Travel

  1. Faculty must complete/do the following:
    • Travel Request Form (1A)
    • Detailed Field Trip Itinerary
    • Field Trip Proposal (if traveling out of the country)
    • Field Trip Memo with Student Emergency Contact Information
    • Obtain Foreign Travel Insurance through department (if traveling out of the country)
  2. Students must complete the following:
    • Student Participation Agreement Form
    • Medical Information and Authorization Form
    • Assumption of Risk and Release Form
